Community Grant Profiles
Descriptions of 2016-17 Grant Programs are as follows:
Allegany County Health Department – Komen Kares Breast Cancer Support Services
Provides transportation assistance and medical assistance for out-of-pocket expenses, including co-pays and deductibles, for Allegany and Cattaraugus County residents diagnosed with breast cancer.
ECMC Lifeline Foundation – Breast Cancer Patient Navigation Program
Provides culturally competent, evidence-based breast health education and screening services to increase the mammography rate among underserved women throughout Allegany, Cattaraugus, Erie and Wyoming counties utilizing the Mobile Mammography Coach.
International Institute of Buffalo’s – Immigrant and Refugee Breast Health Awareness Project
Addresses the cultural and linguistic barriers faced by the immigrant population in Erie County through the use of interpreters, culturally competent health educators, and attention to cultural barriers in the patient navigation process.
Roswell Park Compass Project – Compass Project Breast Cancer Patient Navigation Services
Provides compassionate, culturally appropriate comprehensive navigational services to African American, Hispanic, lower income and rural women who receive abnormal findings on a breast imagining test to obtain timely and complete breast cancer clinical services.
University at Buffalo Research Foundation’s – Patient Ambassadors to Activate Breast Health Continuum Care in Underserved Primary Care Practices
Provides screening events at primary care practices on the East Side and West Side of Buffalo. Uses trained Patient Ambassadors to provide breast health education and assist women through the screening steps.
